authorise the following person
to use my University of Canberra / Library identification card for loan transactions at the University of Canberra Library, and for the collection of Interlibrary Loans / Document Delivery material.
I understand that the person I so authorise will be required to bring their personal photo identification and my University of Canberra / Library identification card to the Library to borrow or collect items on my behalf.
I agree that I will be responsible for all items borrowed on the account while this authorisation is valid. I note that this authorisation is valid until the expiry date of my library membership unless I advise the library otherwise in writing.
I note that the Library will not be held responsible for any usage of the card while such authorisation is valid.
