Learning outcomesOn successful completion of this unit, students will be able to:
1. Articulate health promotion and primary care in the context of chronic and complex care;
2. Identify key self-management principles and strategies;
3. Discuss and practice motivational interviewing skills;
4. Critically analyse care coordination and self- management in the context of the risk of acute hospitalisation;
5. Demonstrate understanding of intensive (focused) care coordination for people with complex, unstable and comorbid conditions during hospitalisation, including early detection and secondary prevention strategies; and
6. Critically reflect on practice in the context of person and care centred care that is evidence informed and framed by communication, integration, resources and collaboration within a multi-disciplinary team.

Submission of assessment items
Extensions & Late submissions
Assessments will be submitted via this units Canvas page. All extensions, moderations and particulars are outlined in the University of Canberra's Assessment policy and procedure.
Special assessment requirements
To pass this unit, students must:
- Attempt all assessment items, and
- Achieve 50% overall.
Moderation will be undertaken for assessment items to ensure that marking is fair and consistent. Moderation processes are in line with the Faculty of Health guidelines and the University Assessment Procedures https://www.canberra.edu.au/Policies/PolicyProcedure/Index/369
Students can apply for an extension to the submission due date for an assessment item through extenuating, evidenced circumstances (specific details are found in the Assessment Procedures). Extensions must be applied for before the due date. Documentary evidence (e.g. medical certificate) will be expected for an extension to be granted, however this will not guarantee that the application will be successful. The Unit Convener or relevant Program Director will decide whether to grant an extension and the length of the extension.
An Assignment Extension form is available via MyUC under ‘Study'.
Late submissions will only be accepted for a limited period. If more than one late submission is made within that period, only the first late submission will be accepted for marking and may be subject to penalties as detailed in the Assessment Procedures.
Late submission of assignments without an approved extension will result in a penalty of 5% reduced marks from the total available, per calendar day late. An assignment submitted over 7 days late will not be accepted.
There will be no supplementary assessments for this unit.
You will note the student workload guideline above (6a).
The scheduling for all units in the Bachelor of Nursing as per Allocate+ indicates that students are expected to undertake up to 5 hours per week per unit of directed/self directed learning (SDL) related to assessment preparation, clinical learning or module content revision.
Each unit has virtual workshop/seminars of which one per module/week will typically be recorded for those who cannot make their scheduled times, this approach supports flexible delivery, recognising the many priorities that students are managing.

It is required that all students participate in all unit learning opportunities in this unit. This is inclusive of all scheduled face to face classes, workshops, and on-line requirements. Assessment items are linked to the unit learning outcomes, Registered Nurse Standards for Practice (ANMC, 2016) and also professional codes of conduct. Workshops/seminar attendance is required to enable students to learn to apply concepts, explore clinical experience in a safe practice environment, and develop and enhance communication skills for safe practice. It also enables students to ask questions, engage in discussion and develop professional behaviours and accountability for safe practice.
