Replacement Certificate

In accordance with the University's policy concerning the issuing of certificates to replace testamurs which have been lost damaged or destroyed, certain conditions (detailed below) must be met before a signed Replacement Certificate can be issued. Use the portal below to order your Replacement Certificate.

If your testamur has been lost or destroyed, please attach a complete (witnessed) Statutory Declaration as Supporting Document below. A Statutory Declaration stating that the testamur has been lost, destroyed or damaged is required. Additionally, an explanation of the circumstances in which this has happened should be provided. In the case of loss, an undertaking should be given to return the replacement certificate in the event of the original testamur subsequently being found.

If your name has changed since your testamur has been issued, please attach a complete Change of Name Form (167Kb PDF) and official documents supporting this name change (e.g marriage certificate, Deed Poll confirmation etc) below.

Return your previous testamur either to the Student Centre (building 1) or via Registered Mail to:

Replacement Certificates will only be issued on return of the previous testamur.

Before proceeding, please ensure you have:

  • Credit/Debit card details to process the replacement fee of AUD $150.00.
  • Your identification number (if you do not have your ID number, please call 1300 301 727) and date of birth.


Before proceeding, you must read and accept the following conditions:

I agree that this request will only be processed after my supporting documentation has been verified by the Manager, Student Progress. Should supporting documentation fail authentication, I understand my request will not be processed until amendments have been made by me. The Student Progress & Graduation team will provide advice on outstanding requirements via email.
