The form is designed so that you can fill it in online, print it out and forward it to your school/college principal.

Do not forget to read our privacy statement and declaration on the form and to add your signature to indicate that you have read it. If you are under 18 a parent or guardian will have to sign as well.

If you cannot fill in the form online, simply print out the form and fill it in by hand.
