How to Apply

Our Application Form is available as a PDF file, for which you will need Adobe Acrobat Reader version 4 or later, or the equivalent plug-in on your browser. Please

  1. fill out the form online, clicking on the appropriate buttons and filling out the text boxes as required,
  2. print it out,
  3. sign and date it by hand and 
  4. send it to UCC with accompanying documentation as requested on the form.

If you need to clear the form and begin again, click on the Clear Form / Start Over button near the space for your signature and the date.

(You may need to download the latest application of the Acrobat Reader from the Adobe website. Go to http://www.adobe.com.au and click on the Download Acrobat Reader icon on the Adobe home page.)

If you are unable to access the PDF version, please use our RTF version for Word on your PC or Macintosh. In this case:

  1. print out the form,
  2. fill it in by hand, sign it and date it and
  3. send it to UCC with accompanying documentation as requested on the form.
