File Action Request Form 

This form can be downloaded and used in Word to produce our standard File Action Request. Fill out the form in Word and then either print the form out and send through the internal mail, or else send it as an e-mail attachment to registry@canberra.edu.au

It is a .dot template file which you download and add to the Templates folder on your hard disk. 

• If you are using Windows 95 or 98 this folder can be found at c:\Program Files\Microsoft Office\Templates
• If you are using Windows 2000 this folder can be found at c:\Windows\Application Data\Microsoft\Templates

Now when you open a new Word document, this template should appear as an option:

Instructions on how to fill out the form have been supplied as Hidden Text in red: if you cannot see these instructions, please go to the Tools menu in Word, select Options and then View. Now click on the Hidden Text checkbox.
