Change or Project Request Form
Proposing a change in the ICT environment requires the submission of a Change or Project Request (Word 146kb), which is analysed by ICT Services and submitted to the Technical Resources Group for resource allocation.

The EPMO is developing project forms, templates and guidelines for each stage of a project, plus companion guides containing instructions and examples for reference when completing the documents.
New Arrival Form
The New Arrival Form is available from the Human Resources website in PDF format (67kb) or RTF format (195kb). It should be completed by all new staff and their supervisors.
Position Change Computer Account Form
Complete the Position Change Computer Account Form (PDF 18kb) to ensure that your changed details are registered on the switchboard database and any other relevant systems.

The Callista Student Management System is available to staff while undertaking their duties related to the operations and mission of the University. Completion of the Callista Student Management System Request for Access form (PDF 18kb) will ensure that your request for Callista Access has been registered so that the appropriate access can then be arranged.
Prophecy Form
